* Students / Residents- All students and residents must submit a signed letter from department chair on the same day as they register. Delegates who do not submit a letter will be charged the higher registration fee.
Conference Fees include: admission to all general session lectures and poster sessions. Lunches on Thursday and Friday. Welcome Reception on Wednesday night. Exhibit Reception on Thursday night. All AM and PM breaks.
Conference Fees do not include: Accommodations, Industry Workshops (Wednesday), CE Workshops (Saturday), Accompanying Guest/s and Banquet (Friday). |

Cancellation Policy: Prior to April 15, 2010, if requested in writing, registration fee will be refunded less a $75 administrative cost. No refunds after April 15, 2010.
